Medical certificate

A medical certificate is issued by your general practitioner or a specialist at the hospital.

A medical certificate must include:

  • Your name and date of birth
  • Your CPR number  (Danish Civil Registration System) if you have one
  • Date of issue
  • Name of the health professional and the institution/organisation,  CBR number/ provider number/ registration number and signature/stamp
  • A clear diagnosis or description of your condition
  • A brief description of any treatment you are receiving
  • A description of how the condition affects your ability to study and complete an education
  • Dates of first and latest consultation, hospitalisation, therapy session etc.
  • Description of whether the professional judgements have been made on the basis of the professional’s examinations or the patient’s statements.
   

Printouts of digital documents must show the URL (website address).

Any cost associated with obtaining a medical certificate is at your own expense.
