Specialization within the healthcare system and the multidisciplinary teams have led to improved treatment of many diseases.
At the same time, patient care pathways increasingly span hospitals, general practice, municipal services, and patients’ private lives.
As a result, today’s healthcare system involves multiple simultaneous areas of collaboration and a range of stakeholders.
For patients, as well as healthcare professionals, this increases the unpredictability of coordination and collaboration, affecting how patient care pathways can be planned.
This research area examines how general practice can contribute to ensuing effective patient care pathways.
