Application form

You can download the application form or fetch it at the Continuing education office at Campusvej 55 in Odense. We are open from 10.00 to 14.00 and will help you fill out the form and inform you about the single course programme in general.

 If you are not already enrolled at the University of Southern Denmark, you must enclose documentation of your qualifying examination and needed entry level. Please also enclose your residence permit if not menber of the EU.

If you don´t have a Danish civil registration number (CPR) fill in like this: ddmmyy-two first letters in your first name, first letter in last surname, 1 for male and 2 for female.
